If you are a shareholder, the majority, if not all forms you would need, are available on the buildings website at

You would need a temporary login name and password to set up your account if you have not done so. That can be requested of the building's property manager, Patrick McGovern either by phone, email or in person. His office is at Citylights in the hallway beyond the elevator bank. He is available Mon, Tues, Wed, Fri 9am - 5pm (lunchtime not included) and Thursdays 2pm - 5pm.
